Thank you for agreeing to be an evaluator in one of our 360° Personal Performance Assessments.
This process is designed to help the participant that you are evaluating become more
effective in a variety of leadership and management behaviors. The key benefit is
that the participant will be able to see himself or herself through the eyes of
others who have worked closely with the participant. The participant you are evaluating is responsible for assigning each of his or her evaluators a specific evaluator token. This token should not be shared with any one else. If, for some reason, you feel the integrity of your token has been compromised, please tell the participant immediately and the token will be destroyed. The survey should take you between 15-20 minutes to complete and contains three parts. You will have three weeks from the time your participant received your token to complete the survey. Please take this survey in a secure environment. If you will be unable to complete the survey, please inform the participant immediately and your token will be reassigned or destroyed. If you are unable to complete the survey in one sitting, you will be able to save it and return another time to finish it. However, to ensure privacy, we recommend that you complete the survey in one sitting. Once you finally submit your survey you will not be able to change your responses.
